The scheduler will confirm that your insurance is accepted and discuss availability with you.

Show up the day of your visit
If you need to cancel your visit or change appointment times, please call as soon as possible and we will reschedule.

What about referrals?
A referral is sometimes required in order to be seen by a specialist. You will need to contact your insurance company to find out if a referral is needed. To obtain a referral, you can contact your primary care provider (PCP), explain your desire to be seen by a pain specialist at Premier Spine & Pain Institute, and request a referral.
Your PCP may require you to come in for a visit, or they may referral you virtually without seeing you. You must ensure that we have the referral prior to your visit, otherwise you will be financially responsible at the time of service.
Copays
Many insurance carriers require that we collect co-pays and/or deductibles from patients. If co-pay is required by your plan, you will be expected to pay that amount at the time of service. Due to federal laws and our contracts with the insurance carriers, we are prohibited from waiving these fees.
